SummaryThe registered nurse in the OR department is responsible for the administration of the total nursing care within the OR physical environment, administrative structure and according to established policies and procedures. Provides optimum patient care through performance and or leadership of subordinate personnel. The RN uses the nursing process to assess the patient, identify desired outcomes, formulates nursing diagnosis, chooses a plan of care, implemnts or delegates inveterventions and evaluates outcomes of the care provided. Among many of the responsibilities, the nurse manages patient positioning, antimicrobial skin preparation, medications, implants, specimens, warming devices, and surgical counts.

Experience/Education Requirements

  • Graduation from an approved school of nursing, BSN preferred. As of 1/1/18 must obtain BSN with 4 years of hire or transfer.
  • Completion of RMC mandatory inservices, upon hire and yearly.
  • Completion of 6 hours of continuing education per year desirable
  • Completion of 6-12 week minimum basic OR orientation with the OR department orientation with preceptors and continued orientation with preceptors.

License Or Certification Requirements

  • Current RN License with the Illinois State Board of Examiners.
  • Current CPR training preferred

Responsibilities

  • Actively reviews the patient's chart and addresses all outstanding issues for IP/OPS
  • Detects and collects comprehensive data and identifies patterns pertinent to the patient's health and unique situation.
  • Effects changes through engagement, involvement, and participation of peers and colleagues, leading others where they need to be rather than where they want to go (American Nurses Credentialing Center [ANCC]).
  • Evaluates one's own nursing practice in relation to the Illinois Nurse Practice Act and professional practice standards guidelines.
  • Implements the identified care plan based on the nursing process, out nursing mission and philosophy, and our organization's mission
  • Assists in room turnover and preparation for cases and terminal cleaning at the end of the day.
  • Completes damp dusting and outdating as assigned before the end of the month. Removes all expired items from circulation.
  • Maintains knowledge of proper use of the equipment and reports malfunctioning devises and takes it out of service and reports it to the desk facilitator.
  • Mentors others in all aspects of nursing practice and participates in precepting of new nurses.
  • Promotes team work through and inclusion of staff in every interaction and training.
  • Proper handling and verification with the surgeion. Places OR Specimen sticker on all specimen containers excluding culturettes
  • Understanding of isolation precautions and protocols for the OR, gowning and gloving as appropriate with proper signage and cart placed outside the OR room
  • Responds to call as assigned within the 30 minute response time. Is mindful to relieve previous shift on time.
  • Take assigned call weeknights (1-2 days), weekends (every 3rd to 5th weekend) and holidays on a rotation basis.
  • Follows proper medication administration following the 7 rights. Verifying expiration date with scrub staff and ensuring proper labeling as it is being given onto the sterile field.
  • Maintains aseptic technique through gowning and gloving.
  • Assists surgeon by second scrubbing into cases and passing instruments and draping appropriately.
  • Opens sterile supplies properly and helps to set up the back table
  • Recognizes instruments and trays and can help to identify what is needed in a specific procedure

  • Reviews the communication book updates when unable to attend morning meeting.
  • Promotes team work and the inclusion of new staff or other staff training in one’s specialty.
  • Utilizes the “CUS” acronym and observes and reports pertinent patient information to the charge nurse or circulating RN.
  • Ensures patient safety is the primary priority.
  • Mentors others with strict adherence to policy, procedures, and behaviors.
  • Understands and utilizes the error reporting system is not punitive, but a tool to promote process improvement
  • Understands that role is not limited to what is listed in the job description and that team work is defined by doing whatever is necessary to promote efficiencies and provide safe patient care.

Vision: Required to assist surgeons with scrubbing and circulating. Many OR procedures require keen visual observation because of the size of suture material used/instrumentation used. It is necessary to perform patient assessment, medical administrations, observe video monitors, etc.

Smell: Helpful to note presence of incontinence, vomitus, blood; electrical/fire safety.

Hearing: Necessary to hear patient's concerns, to hear surgeons during procedures, to hear intercom for help in a room, hear alarms, i.e., fire, code 99, etc.

Touch: Necessary to comfort patients, hand instruments during procedures necessary to operate controls on equipment.

Lifting equipment - carry 25'-150' waist high (IV bags, patient trays). Carry charts 25'-150' waist high. Filled laundry bags are filled half full so as not to weigh over 35 lbs. Carry fire extinguishers up or down 3-4 flights of stairs and several yards in an emergency.
